Community Health Foundation of Puerto Rico (CHFPR) selects MediVisum© for Its COVID-19 Mobile Outreach Program’s Telehealth Patient Care Platform

CHFPR, has partnered with MediVisum as its telehealth platform to support its COVID-19 Community Outreach Programs. MediVisum provided the security, patient privacy, and documentation functionality that they needed in general and their clinically based tools were easily configurable to CHFPRs specialized workflows associated with the Covid-19 Mobile Outreach Program.

According to Dr. Ismael Toro Grajales, the Board Chairman at CHFPR, “It was very important to us to have an application that would support our outreach efforts to patients who had difficulty accessing our center during the pandemic, and help drive our community-based Covid-19 Mobile Outreach initiative. This initiative was designed to reduce the COVID-19 transmission rates among high risk r populations by having teams of trained healthcare professionals move across our service region to comprehensively screen and triage patients at their homes as well as in residential settings such as homes for the aged.
